Pre-Appointment Checklist
To be prepared for your appointment, please bring the following:
• Completed Patient Registration Packet
• Health Insurance Card/Information
• A list of all the medications you are taking along with the dosage and frequency
• A list of all the questions and concerns you may have
• Any diagnostic imaging studies you may have (x-ray, CT, MRI)
• Copies of records from any earlier treatments in the past
• A family member or friend to take notes or help with any questions you may have
