Insurance & Workers Comp
Insurance
We bill most insurance companies for you when the proper paperwork including insurance cards, is provided to us. We will also bill most secondary insurance companies for you. If you have concerns about pre-authorization, please contact your insurance company.
Workers Compensation
Your treatments may be paid for by your employer if you've been injured on the job. If your visit is due to a work related injury, prior approval from your Workers Compensation case manager must be obtained. We will need the claim number, carrier name, and information prior to your visit. The patient is ultimately responsible for all professional fees if a Workers' Compensation claim is denied. Spine & Musculoskeletal Medicine also provides second opinions and Comprehensive Record Reviews.
Medicare Patients
We will bill Medicare for you. We will also bill secondary insurance carriers for you.
Welfare Patients
All Welfare patients must provide a current valid card before being seen.
Liability Claims & Auto Accidents
A required $350.00 payment is due at the time of service for initial evaluation and treatment.
Self Payment Accounts
We ask that you make payment at the time of service. A minimum of $350.00 payment is due at the first visit and $150 for each follow up at the time of service. We are happy to accept payment by cash, check, or credit card, If your account reaches 90 days past due and you have not contacted us to make payment arrangements, your account may be turned over to a collection agency.
