Thank you for choosing Spine & Musculoskeletal Medicine for your musculoskeletal care. We look forward to providing your care. Here are some of our requirements and policies that you should be aware of.
Appointments
Please print and complete the necessary forms prior to your visit. If you have problems completing the paperwork, please come at least 30 minutes early for assistance. Bring a current insurance card and a photo ID. Co-payments are due at the time of the service. Bring a current list of medications you are taking. This will need to be provided at each visit.
We will make every effort to make sure we have your medical records and other information related to your current condition. However, we may need to contact you for this information.
If you are more than 15 minutes late for your appointment or do not have the required paperwork for your appointment, you may be asked to reschedule.
